Summary
This commenatry on Rosenthal, Wyatt, and McMahon’s review of cognitive and behavioral therapies (CBTs) for personality disorders (PDs) discusses implementation challenges to cognitive and behavioral approaches in real world settings. Multiple interventions show some empirical support for the treatment of PDs, including dialectical behavior therapy (DBT), schema-focused therapy (SFT), cognitive therapy for personality disorders, emotion regulation group therapy (ERGT), and systems training for emotional predictability and problem solving (STEPPS). Most of this work is restricted to borderline personality disorder (BPD) and DBT, whereas strong empirical support for other PDs is lacking. Here the authors discuss possible factors that may account for differential research support for the treatment of BPD relative to other PDs. For example, the BPD diagnosis includes suicidal and self-injurious behaviors among its criteria and is associated with DBT and its relevant training organizations and adaptations across multiple levels of care. Additionally, they provide examples in which interventions have been implemented ahead of the science. Finally, they discuss challenges to implementing CBTs for PDs in real world settings, such as diagnostic heterogeneity, comorbid presentations, and dual diagnosis, as well as the longstanding nature of problems associated with PDs.
Parsing neurodevelopmental features of irritability and anxiety: Replication and validation of a latent variable approach

Irritability and anxiety are two common clinical phenotypes that involve high-arousal negative affect states (anger and fear), and that frequently co-occur. Elucidating how these two forms of emotion dysregulation relate to perturbed neurodevelopment may benefit from alternate phenotyping strategies. One such strategy applies a bifactor latent variable approach that can parse shared versus unique mechanisms of these two phenotypes. Here, we aim to replicate and extend this approach and examine associations with neural structure in a large transdiagnostic sample of youth (N = 331; M = 13.57, SD = 2.69 years old; 45.92% male). FreeSurfer was used to extract cortical thickness, cortical surface area, and subcortical volume. The current findings replicated the bifactor model and demonstrate measurement invariance as a function of youth age and sex. There were no associations of youth's factor scores with cortical thickness, surface area, or subcortical volume. However, we found strong convergent and divergent validity between parent-reported irritability and anxiety factors with clinician-rated symptoms and impairment. A general negative affectivity factor was robustly associated with overall functional impairment across symptom domains. Together, these results support the utility of the bifactor model as an alternative phenotyping strategy for irritability and anxiety, which may aid in the development of targeted treatments.
Association between maternal vitamin D status in pregnancy and neurodevelopmental outcomes in childhood: results from the Avon Longitudinal Study of Parents and Children (ALSPAC)

Seafood intake in pregnancy has been positively associated with childhood cognitive outcomes which could potentially relate to the high vitamin D content of oily fish. However, whether higher maternal vitamin D status (serum 25-hydroxyvitamin D (25(OH)D)) in pregnancy is associated with a reduced risk of offspring suboptimal neurodevelopmental outcomes is unclear. A total of 7065 mother–child pairs were studied from the Avon Longitudinal Study of Parents and Children cohort who had data for both serum total 25(OH)D concentration in pregnancy and at least one measure of offspring neurodevelopment (pre-school development at 6–42 months; ‘Strengths and Difficulties Questionnaire’ scores at 7 years; intelligence quotient (IQ) at 8 years; reading ability at 9 years). After adjustment for confounders, children of vitamin D-deficient mothers (<50·0 nmol/l) were more likely to have scores in the lowest quartile for gross-motor development at 30 months (OR 1·20; 95 % CI 1·03, 1·40), fine-motor development at 30 months (OR 1·23; 95 % CI 1·05, 1·44) and social development at 42 months (OR 1·20; 95 % CI 1·01, 1·41) than vitamin D-sufficient mothers (≥50·0 nmol/l). No associations were found with neurodevelopmental outcomes, including IQ, measured at older ages. However, our results suggest that deficient maternal vitamin D status in pregnancy may have adverse effects on some measures of motor and social development in children under 4 years. Prevention of vitamin D deficiency may be important for preventing suboptimal development in the first 4 years of life.
D-cycloserine Inhibits Amygdala Responses During Repeated Presentations of Faces

Introduction: Recently, human studies using exposure therapy to treat anxiety have demonstrated that pretreatment with D-cycloserine (DCS) enhances fear reduction in anxiety disorders. However, the underlying brain mechanisms mediating this fear reduction have yet to be determined.
Methods: The effects of orally administered DCS on amygdala activity during the processing of repeated facial expressions were examined in this double-blind study. Fourteen healthy males (30.0±8.7 years of age) randomly received DCS 500 mg or placebo prior to 3.0 Tesla functional magnetic resonance imaging acquisition. All participants viewed four separate runs, consisting of a single block of a repeated facial expression (happy or fearful) bracketed by fixation blocks.
Results: Anatomic region-of-interest analyses showed that the placebo group exhibited amygdala activation and response habituation, while the DCS group displayed blunted amygdala responses to emotional faces across the experiment, whereby habituation was not detected.
Conclusion: This finding may have relevance for testing treatments of anxiety and depression.

Summary
This chapter describes a set of new directions in the understanding of how poverty and poverty-related individual and environmental risks influence children and families. It presents the definitions of poverty and of the environment, and discusses new and emerging multidiscipline research on the links between poverty and child and family outcomes with an emphasis on mechanisms. The chapter summarizes the current state of the evidence emerging from the basic sciences (e.g., neuroscience), psychology, sociology, and economics. It outlines the innovations in prevention and intervention efforts both to change poverty itself and to interrupt the links between poverty and poor child and family outcomes. Some of the most innovative poverty reduction or alleviation interventions are place- or setting-based, seeking to infuse entire communities or settings with resources meant to reduce the prevalence or impact of one or more poverty-related risks.
